Verification Form Feature

How to use the verification form feature from your uSked site & how it is displayed for service providers in the uSked app

The Verification Forms feature allows service providers to collect a signature and upload a form or photo. It enables service providers to meet the verification needs of your contracts from within the app.

The Take a Photo button allows you to do just that. Take a photo and then you’re able to use the Upload a Picture function for the actual upload. Upload a File will pull up your files, allowing you to select which to upload. The Fill & Sign function enables your service providers to fill out the necessary paperwork on the app, including the option for a signature.

⚠️FILL & SIGN OPTION⚠️

There are several tools available within the Fill & Sign option. Here is a key to what these tools can do:

Signature Tool

This tool allows service providers to use their phone to allow for finger sign signature capture. Once the signature has been confirmed the service provider can then move the signature on the form to place it in a signature box.

Text Tool

This tool allows service providers to type in text on a verification form. Once they tap "return" they can then move the text to any location on the verification form.

Dot Tool

This tool allows service providers to place a dot anywhere on the verification form. This is most commonly used to fill in a circle.

Check mark Tool

This tool allows service providers to place a check mark anywhere on the verification form. This is most commonly used to fill in a checkbox.

X tool

This tool allows service providers to place an "x" anywhere on the verification form. This is most commonly used to fill in a box.

Pre-populated information from the assignment

Service providers can add pre-populated information to a verification form through a quick three-step process.

Step 1 From the Fill & Sign option, open the pre-populated information selector by tapping on Event Name in the lower left-hand corner. Then tap on which type of information you'd like to add to the form.

Step 2 Next, tap on the "+" next to the selected field.

Step 3 Then tap on the form where you'd like the information to be added.
