How to place a request

Step 1

There are two ways you can begin to create a new request – either option will get you to the request entry.

Begin by selecting “Create a New Service Request” on your home page, or you can click: Scheduling > Create a New > Service Request from the main menu.

Fill out all of the Basic information.

If you are requesting services for yourself, select “Yes” for the option to “Add Yourself as Client Receiving Services.” Select “No” if you are requesting services for someone else.

If your request has been created successfully, you will see a System Alert appear. Click “Ok.”

Step 4

Fill out all of the required information in each of the tabs at the top of the form. When you are done, click “Save Changes and Submit as Pending Confirmation.” If you need to stop and come back before submitting, you can click “Save Changes and Keep as Draft.”

Step 5

Fill out all of the required information in each of the tabs at the top of the form. If your request has been submitted successfully, you will see a System Alert. Click "Ok."

Step 6

A scheduler will review your pending request and initiate the search for service providers if everything looks good. From this page, you can track the status of your request.
